This survey is intended for members of Positive Women’s Network – USA, defined as any self-identified woman living with HIV, including people of trans experience, who wishes to be a member.
In order to enter the raffle for a $100 gift card, you will need to complete the survey below in full. Once you have answered all the questions, you will get a message that the survey is complete, with a link to click to take you to a separate form to fill out your contact info in order to be entered in the raffle. This process is to ensure the anonymity and confidentiality of your survey responses. As a result, unless you complete that form with your contact info, we will not be able to enter you in the raffle.
